When tetracycline was first introduced as a new antibiotic option in the early 1950s, it was greeted with lots of fanfare. It was great to have a new antibiotic option for doctors to use with patients, especially if a patient reacted to other antibiotics. Unfortunately, tetracycline almost immediately started to show of pattern of staining teeth. The first case of reported tooth discoloration in children occurred in 1956. Before the link could be made many, many other children had stained teeth over the following decade.
Tetracycline stains do not cause any harm to your teeth, but their dark colour can affect your smile and self-confidence. The discoloration usually occurs when antibiotics are prescribed at a young age – between 3 and 10 years – when the teeth are still developing.
The staining occurs inside the tooth, such that its least visible on the surface of tooth enamel, but gets darker towards the pulp. With the placement of porcelain veneers, the dentist must first grind away part of the tooth enamel to create room to bond the veneers. Unfortunately, this also makes the discoloration more visible and harder to hide with veneers.
Veneers, because of their reputation, are often sought after to correct tetracycline staining. However, for severe cases, the veneer being so thin, extremely dark stain might show through. When the stain is that dark, slightly more tooth structure must be removed to give enough room for the porcelain to block out the stain.
Severe Tetracycline stains are one of the most difficult types of cosmetic cases a dentist can work with. They have to make the porcelain opaque enough to cover the stains while translucent enough to still look natural.
Depending on the severity of discoloration, there are several treatment options including vital and nonvital bleaching, micro abrasion, composite and porcelain veneers, porcelain crowns and sometimes a combination of them. The vital bleaching procedures cannot satisfactorily remove dark tetracycline staining. For decades, severe intrinsic staining has been often treated with crowns, but this supposes the aggressive elimination of dental structure. Currently, several types of all-ceramic systems have been developed to satisfy patients’ aesthetic demands.
