For dental teeth bonding, the dentist will first remove the decayed or damaged portions of the tooth. The reduction will be very minimal. Much like dental bonding, porcelain veneers are also fused to the tooth structure. The only difference is instead of using composite (plastic), ceramic (glass) is used. Also, the amount of tooth reduction is much greater. The dentist will have to remove structure about 0.5mm on in the entire front surface of the tooth in order to accommodate the veneer.

Dental bonding procedures are performed entirely in a dental clinic. With porcelain veneers the tooth preparation and placement of the restoration for porcelain veneers are performed in the dental clinic. The actual veneer is fabricated in a laboratory by a dental technician.

The porcelain veneers produced by dental laboratories mimic the natural teeth. Technicians utilize layers of porcelain in different shades to get the best aesthetic possible. That is very difficult to do with dental bonding. Even though dentists use different colours of composite to copy the natural shade of the tooth, the results are not as good.

For kids, dentists typically like to stick to dental bonding mainly because it is less invasive and anaesthesia is not necessary most of the time. More importantly, kids are growing and so is their bone structure. Many things change over time and the last thing parents need is to spend a lot of money for veneers on someone that is still growing. When they reach adulthood, the veneers may not look as good and need to be redone.

Furthermore, the oral hygiene habits of a kid are not as good as an adult and the veneer may fail because of that. Porcelain veneers are an investment and a person wants to make sure they last for as long as possible.

That is where another misconception comes in. If a dentist tells patients that porcelain veneers will last for 15 years, they think the veneers will just fall off after 15 years. That is not the case. Veneers do not come off for any reason. They get removed either because they fractured or there is leakage and recurrent decay underneath.

How do Dental Bridges Work?

 

A Dental Bridges may be recommended if you’re missing one or more teeth. If you are missing one or more teeth, the gap in your smile can cause many other problems, including additional decay, sensitivity, tooth drift, gum disease, and a change in your bite . Gaps left by missing teeth eventually cause the remaining teeth to rotate or shift into the empty spaces, resulting in a bad bite. The imbalance caused by missing teeth can also lead to gum disease and in many cases, too, a missing tooth can cause the temporomandibular joint, or TMJ, to become misaligned, a condition referred to as TMD.

A Dental Bridges is a restoration designed to fill the gap caused by a missing tooth and prevent additional dental problems. Dental Bridges are cemented to the natural teeth or implants surrounding the empty space. These teeth, called abutments, serve as anchors for the bridge. A replacement tooth, called a pontic, is attached to the crowns that cover the abutments. As with crowns, you have a choice of materials for bridges, based on the location of the missing tooth (or teeth), its function, aesthetic considerations and cost. Porcelain or ceramic bridges can be matched to the color of your natural teeth.

Are Fixed DENTAL BRIDGES NECESSARY?

 

Many people who lose one or more back teeth are often inclined to do nothing to replace them. Unfortunately, losing a tooth can start a vicious cycle of destruction. In a normal, healthy mouth, your teeth have a natural balance. Each upper tooth contacts the corresponding lower tooth, allowing for a well-balanced chewing function. Losing a tooth quickly destroys this balance. A functional mouth has 28 teeth (14 upper and 14 lower – excluding wisdom teeth). They work as a team. Each tooth has three companion teeth: the two adjacent teeth and the opposing tooth. These companion teeth provide support and stability. Drifting, shifting, periodontal disease, and abnormal decay areas are some of the problems that can occur when your tooth loses the support of a companion. The adjacent teeth shift inward toward the missing space. This causes open spaces between the adjacent teeth and their companion. The opposing tooth will drift down into the space left behind by the missing tooth. This causes interferences which will affect eating and talking. As the opposing tooth drifts out of its socket, the bone support is lost and the tooth will inevitably be lost as well. This cycle is a domino effect which will lead to the loss of many otherwise healthy and vital teeth.

  • The unprotected ridge is subject to trauma.
  • The neighbour teeth tilt, destroying proper functional balance.
  • Shifting teeth create spaces which food will become trapped and lead to decay and periodontal disease.

What to Expect with a Dental Bridges?

 

The process of having a dental bridges inserted is a pretty simple and painless procedure. Usually, you will need to visit your dentist at least twice for this process. During the first appointment, your dentist will prepare the teeth on either side of the gap. If they are to be covered in crowns, your dentist will file away some of the enamel, reshaping the tooth to create space for the crown. He or she will then make an impression of your teeth to send off to a lab.

The lab will use this impression to create a custom Dental Bridges for your mouth, which your dentist should receive within a few days. In the meantime, your freshly prepared teeth will be covered with temporary crowns to prevent sensitivity and damage until your bridge comes in. You should not feel any pain during this appointment because your dentist will inject a local anaesthetic to numb the nerves in your teeth.

At your second appointment, your dentist will remove the temporary crowns, if present, and then cement your Dental Bridges into place. In some cases, your dentist may first use a temporary cement. This allows you to evaluate the fit and feel of the bridge in your mouth over the coming weeks. You’ll return for a third appointment during which your dentist will talk to you about the fit, make changes if needed, and then permanently cement the bridge into place.

 

Procedure in Making Dental Bridges

 

We prepare your teeth on either side of the space for the false tooth. Teeth on either side of the space are prepared to accommodate for the thickness of the crown. We will then make an impression, which will serve as the model from which the dental laboratory will be make the Dental Bridges (false tooth and crowns). A temporary bridge will be placed for you to wear while your bridge is being made until your next visit. This temporary bridge will serve to protect your teeth and gums. On your second appointment, the temporary bridge will be removed. Your new permanent bridge will be fitted and checked and adjusted for any bite discrepancies. Your new bridge will then be cemented to your teeth.

 

Caring For Your Dental Bridges

 

A dental bridges depends on the health of the adjacent teeth and gums for support. To care for your bridge, brush and floss normally after each meal.

Superfloss and floss threaders are effective tools for keeping the area under your bridge plaque-free. Superfloss has a stiff end that helps in threading it through tight areas and a fuzzy tufted segment that can remove plaque as you floss. Insert the superfloss under the bridge and use it to floss the sides of the teeth and under the bridge. Floss threaders also aid in removing plaque. Pull out about 18 inches of floss, insert it through the floss threader, and then use the threader to insert the floss under your bridge. Floss to remove food particles and plaque from the sides of the teeth and under the bridge. Use the same procedure to care for your temporary bridge, being careful not to dislodge it as you brush and floss. If it does come loose, gently re-attach it; call your dentist if your temporary bridge frequently comes loose.

 

Recovery and Dental Care After a Dental Bridges

 

You might have a little sensitivity in the teeth to which your bridge is attached in the days following your appointment. This is just because your dentist has activated the nerves in these teeth while working on them. The sensitivity should wane in the days that follow. Many patients find that chewing and speaking with a bridge is a little awkward at first, especially if the teeth have been missing for some time. However, the body adapts quickly, and before long, you will be able to chew on your bridge just like you would a natural tooth.

With proper care, a dental bridges can last 15 years and often much longer. Here are some ways to properly care for your dental bridge:

  • Brush your teeth twice a day for at least two minutes, being sure to brush around the bridge just like you would a natural tooth.
  • Floss between the bridge and your gums to remove oral bacteria that accumulate in this area and may cause gum disease. Your dentist or dental hygienist can show you how to do this.
  • Use an antiseptic mouthwash to help kill germs in the tissues surrounding your bridge.
  • Visit your dentist for regular checkups to ensure any issues with your bridge are detected early.

There are no serious dietary restrictions that you must follow when you have a dental bridge. However, you should minimize your intake of overly crunchy and chewy foods, as these place excess strain on the teeth supporting the bridge and may contribute to shifting of these teeth over time. If you eat something sticky or gooey, try to chew on the opposite side of your mouth from the bridge.

If you ever begin to experience pain or aching in the teeth to which your bridge is attached, contact your dentist. Also see your dentist if the gums around your bridge become swollen, sore, red, or prone to bleeding. These are signs of gum disease that must be addressed to prevent decay in the teeth to which your bridge is attached.

COMMON DENTAL CROWN PROBLEMS AND THEIR SOLUTIONS

1. Tooth decay

Can your teeth rot under a crown? Yes, it can.  Of course, it is not the crown that gets the decay, but the tooth.  When this happens, the decay quickly spreads underneath the crown.  Usually there is metal underneath the porcelain or the crown is made entirely of metal.  This metal makes it impossible for the tooth structure inside of the crown to be seen on a radiograph (x ray).

Depending on the size of the tooth involved, there can be precious little tooth structure left under the crown before the pulp (nerve) is destroyed by the decay.  If this happens, a new crown will not fix the tooth.  Under these circumstances, a root canal will be needed along with a new crown—assuming the tooth is able to be saved at all.  Additionally, a new crown on a tooth that has already had a crown will without question be much more difficult to do than the first crown.  This is because the margins (edges of the crown) will need to be deeper to cover up the area where the decay had to be removed after the old crown was taken off.

In conclusion, crowns are long lasting.   Decay under a crown is a common way that a crown eventually fails.  Early detection of decay at the edges of a crown reduces the amount of destruction of tooth caused by decay.  A high degree of diagnostic skill is needed to detect early decay on the margins (edges) of a crown.  Timely replacement is needed in order to avoid a root canal, or even to retain the tooth when decay is detected under an existing crown.  Ask your dentist how you can maintain your crowns for the longest time possible.

How to Avoid Getting Decay under your Crown?

The easiest way to avoid having a crown get decay underneath it is to pay special attention at home to the margins (edges) of the crown where it attaches to the tooth.  Brush and floss are mandatory for long life of a crown.  As well, special cleaning tools may be needed.

Prevention is better than a cure in almost every context regarding dentistry, keeping on top of your oral hygiene can save you thousands in procedures.

2. Dental Crown fall off or came out

Sometimes, crowns fall off. Usually this is due to an improper fit, a lack of cement, or a very small amount of tooth structure remaining that the crown can hold on to because of recurrent decay under the crown. If this happens, clean the crown and the front of the tooth. You can replace the crown temporarily using dental adhesive or temporary tooth cement that is sold in stores for this purpose. Contact your dentist’s office immediately either for recementation of your old crown if it is still salvageable or a new crown might be needed.

 

3. Poor crown placement

Another common crown issue is improper crown placement over the affected tooth. In some cases, a poorly fitted crown will have an effect on your bite. When you clench your teeth, you may notice that your crown is uneven, resulting in a wonky bite that can cause discomfort in the future. Only a dentist can address and resolve this issue. Furthermore, as previously mentioned, poor crown placement can lead to bacteria creeping in and affecting the tooth, which can cause discomfort and necessitate costly repair work.

4. Teeth that are sensitive

It is not uncommon for your tooth or teeth to become sensitive following your dental crown procedure. You may discover that eating certain foods and the temperature of food and drink affects your tooth sensitivity.

If the sensitivity persists after the first week or so, there could be another cause. If your dentin is exposed during the procedure (this occurs when your tooth enamel is trimmed away during the crowning process), it may indicate that your crown does not cover your entire tooth.

The tooth under your crown can still become fractured. If you’re experiencing severe pain, there could be a fracture, small crack, or cavity in the tooth beneath the crown.

When a new crown is placed, the biting surface of the tooth changes. A crown that’s just a little too high or off just a bit in fit can result in tooth pain. If you have pain when you bite down, you may simply need to have the biting surface of your crown adjusted.

Receding Gums –

Over time, your gums may recede around the dental crown. This can expose sensitive areas of your gums and a bit of the tooth root, which can result in discomfort and increase your chance of infection

Infection –

If your crown was placed on an existing tooth without doing a root canal, nerves and connective tissue inside your tooth are still there. The connective tissue and nerves in the center of your tooth are known as the tooth’s pulp. If infection occurs in the pulp, it may cause severe tooth pain.

When Should You See Your Dentist?

If you’re only dealing with mild discomfort after the placement of a crown, it will probably subside on its own. However, if you’re dealing with more bothersome tooth crown pain, you should visit to your dentist.

Knowing if your crown pain constitutes a dental emergency will require consulting with your dentist. If there’s a problem with your bite or the fit and alignment of your crown, it’s something that can be easily corrected by a dentist to eliminate your pain. If receding gums around your crown are causing pain, extending the crown may be an option that solves the problem. If you’re dealing with an underlying infection of the tooth pulp, then the crown may need to be removed and a root canal performed. For a cracked or fractured tooth, your dentist may be able to repair it, or you may need an alternative like an implant or bridge if the tooth can’t be saved.

When it comes to crowns, experiencing moderate to severe pain is not normal. Make sure that you schedule an appointment with your dentist to assess the situation as quickly as possible. Your dentist will be able to determine the right solution to relieving your pain and restoring your smile.

7. Dental crown failure due to breakage, cracks, fractures

Dental crowns made from porcelain and can fracture just like your natural teeth, making cracked or fractured crowns one of the more common problems people experience.

The breaking or fracturing of a dental crown is directly related to what the crown is made of.

When a tooth is prepared for a crown it has to first be shaved down (we call it “prepped”) to remove decayed or damaged parts of the tooth. We also need to make sufficient room for the materials that will be used to construct the crown.

There are quite a few different choices for the materials with which the crown is constructed. Each material has its own specifications for its required thickness. This is so that the crown can be strong enough to withstand the stress of chewing and biting on it. It stands to reason that if the dentist uses a type of porcelain which needs to be 2mm thick in order to achieve enough strength, then reducing the tooth by only 1mm would place the finished crown at risk of possible breakage from chewing or biting. The crown would be strong enough for chewing most things but may not be strong enough to withstand the sudden stress of encountering a cherry pit in a cherry pie, or some other unexpected hard substance in your food.

What happens if my crown chips or breaks? : By applying resin to the affected areas, small chips and fractures can be repaired this way. Larger breaks and chips, on the other hand, are likely to be too difficult or complex to repair, necessitating a full crown tooth replacement.

8. Dental crown with a dark line

A dark line next to the gum line of your crowned tooth is normal, particularly if you have a porcelain-fused-to-metal crown. This dark line is simply the metal of the crown showing through. While not a problem in itself, the dark line is cosmetically unacceptable and your dentist may have to replace the crown with an all porcelain or ceramic one.

Natural teeth have a certain level of translucence, since the enamel is composed of light-emitting materials. When crowns are too opaque (solid in color), they don’t emit light. As a result, they can look fake next to natural teeth.

10. An Uneven Bite

Everyone’s bite is unique, and while labs do their best to achieve a perfect bite, it is common for the crown to need adjusting after it has been cemented in place. When you are numbed for crown placement, it can be difficult to get your teeth to bite together normally because they can feel funny. You may need to adjust your crown after the cementation visit once the numbness wears off.

Solution: If you notice that your crown is interfering with your bite, contact your dentist to have the bite and fit of your new crown checked.

11. Mismatched Colours with your Natural teeth

Dental crowns can be made in a variety of unique colorations to match different people’s teeth, and having one that isn’t the same colour as the rest of your teeth can make you look out of place.

  • Using your teeth as tools: never use your teeth as tools, they are never one. Try as much as possible not to use your teeth to open packages, bite your fingernails, tear off tags from clothing, and open crown corks. Doing these can damage both your natural teeth and dental crowns.
  • Brushing and flossing regularly: dental crowns do not require special oral care. They can be cared for just like your natural teeth. Therefore, brushing and flossing at least twice a day with a soft-bristled brush and non-abrasive fluoride toothpaste can make your dental crown last longer.
  • Schedule a regular appointment (or at least every six months) with your dentist for dental cleanings and check-ups: This practice is important whether you have a dental crown or not.
  • Avoid dark-coloured foods and drinks: most dental crowns are made from high-grade porcelain that is resistant to discolouration and staining. However, porcelain crowns can take on a slightly different colour at their edges over time due to the consumption of dark-coloured foods and drinks. Contrariwise, the dark-coloured foods and drinks may change the shade of your natural teeth causing them to look darker or yellowish than the dental crown.

By carefully following the above guidelines, you would be protecting your dental crown and preserving your investment.

Like stated above, dental crown wears in the long run due to daily and the amount of wear is peculiar to each person. Although a dental crown is durable, it is not indestructible i.e. it can be damaged or destroyed over time. Hence, dental crowns need to be replaced if they crack or chip.

Leaving a damaged, cracked, or chipped dental crown in your mouth can increase the possibilities of having dental caries under the porcelain crown. Although dental crowns cannot get decayed, dental cavities can form under them if good and proper oral hygiene practices are not followed. So, depending on the extent of the dental decay and the condition of the dental crown, a dental crown replacement may be necessary. If the dental decay is not properly and immediately taken care of, the underlying tooth may become severely damaged and require a root canal treatment.

A poorly fitted crown, misfit crown, and imbalanced bite are common causes of porcelain crown failure and can also be reasons why permanent porcelain crowns would fail earlier than expected. Who Should Not Undergo Teeth Whitening or Bleaching?

 

It is not advisable to undertake whitening procedures under these circumstances

  • Gum disease, damaged teeth, cavities, and exposed roots. People with damaged teeth or worn enamel are not advised to use any bleaching method. Bleaching does not work on roots also since they do not have the enamel layer on which bleaching acts.

  • Heavily stained teeth. A light yellowish tint will definitely disappear under bleaching; less effective results would appear on brown tinted ones where as grayish or purple stained ones may not reveal any effect. How does teeth whitening work?

 

The peroxide ingredient in most teeth whitening products is what actually bleaches the enamel and makes your teeth whiter. The strength of the whitening treatment typically depends on the strength of the peroxide contained in the whitening product. The current percentages of peroxide in teeth whitening products are 10, 16 and 22 percent. The higher the percentage of peroxide, the higher the sensitivity and shorter wearing time.

The bleaching gel is usually hydrogen or carbamide peroxide that breaks down into oxygen molecules. As the stains are broken into smaller and smaller pieces, more light passes through the teeth and makes them look lighter. Stains that are accumulated over time that are yellow to slightly brown produce the best results. Teeth that are dark brown to blue-gray are the most difficult to bleach because the stain is deeply embedded into the tooth’s structure. These types of stains are usually caused by medications taken during the development of the teeth such as tetracycline, and can take a minimum of three weeks to see results, and as much as six months for complete results.

Depending on the bleaching gel used and the length of time it is worn, results can be seen within an hour, overnight or within three to four days. Complete results can be achieved in as little as a week or as much as several months depending on the source of the stain and the ability of the teeth or tooth to be bleached. The whitening procedure may last for up to three years — even longer in some cases, depending on personal habits. To brighten the color back up, a simple process of bleaching for one or two nights with a single syringe is usually sufficient.

How much discomfort and sensitivity is there?

 

The amount of discomfort experienced varies from person to person, and depends on the method used. Most adults usually experience little if any discomfort, however, the discomfort should subside within 24 hours. If needed, an over-the-counter anti-inflammatory may help. If the discomfort lasts for more than 2 – 3 days, consult your dentist for more options.

  • Decreasing the sensitivity may be as easy as placing a gel fluoride, under your dentist’s instruction, in your bleaching tray for an hour or so. Other materials may be used to address the sensitivity you may experience. Check with your dentist for the best options for your paticular needs. You should discuss your goals, budget, and pain tolerance with your dentist in order to help him/her determine the procedure, or combination of procedures, that will produce the best results for you. If your teeth are sensitive after bleaching, a mild analgesic will usually be effective to make you more comfortable until your teeth return to normal.
  • GUM IRRITATION: Temporary inflammation of your gums can be caused by bleaching. This is the result of very small amounts of bleach leaking over the tray. A burning sensation in your gums may also occur. This is a minor problem and will resolve by itself in a few days. Irritation can also result from using the bleaching tray too many hours when you first start bleaching or using the bleaching tray too many hours in a row without a break: It may be necessary to reduce the number of hours you are wearing the bleaching tray or stop using it for a short time to resolve these gum problems.
  • SORE THROAT: If you overload the bleaching tray with home bleach, the excess can go down your throat and cause soreness. This soreness will also resolve in several days, assuming you spit out the excess bleach instead of swallowing it.
  • LEAKING FILLINGS OR CAVITIES: Most bleaching is indicated for the outside of the teeth (unless you already had a root canal. However, if you have any fillings that are leaking and allow the bleach to get into the inside of the teeth, damage to the nerves could result. In this case the filling needs to be redone prior to the bleaching. In addition, open cavities can also allow bleach to reach the nerves of the teeth. They should also be filled before bleaching.
  • CERVICAL ABRASION/EROSION: These conditions affect the roots of the teeth when gums recede, these are grooves, notches or depressions where the teeth meet the gums that look darker than the rest of the teeth. They look darker because there is no enamel in these areas. Even if these areas-are-not-sensitive, bleach can be potcntially penetrate the teeth and damage the nerves. Those areas should not be bleached, but should be filled after the bleaching is completed.
  • ROOT RESORPTION: This is condition where the root of the tooth starts to dissolved, either from the inside or outside. Although the cause of the resorption has been determined, studies have shown that its incidence is higher in teeth that have had a root canal and are bleached.
  • EFFECT ON FILLINGS: Even though open cavities should be filled or badly leaking filling should be refilled prior to bleaching can cause tooth coloured filling to become softer and make them more susceptible to staining. Therefore, you should be prepared to have any fillings in you front teeth replaced after bleaching. In addition, since bleaching will normally lighten teeth, but not fillings, you may need to have your fillings replaced anyway, so that they will match your newly whitened teeth.
  • TAKE NOTE: Tooth colored fillings, crowns, bridges and veneers will not lighten when your teeth are bleached. You’re Responsibilities
  • WEARING YOUR BLEACHING TRAY: It will only be effective if you conscientiously wear the bleaching tray for the prescribed number of hours per day.

  • The whitening procedure can be very effective at whitening the teeth, but will not change the colour of fillings or crown(s) already in or on your teeth. For aesthetic reasons, such fillings may need changing after the whitening procedure.
  • The whitening treatment plan has been reported not to be effective on some patients. Our office will provide our best efforts to whiten your teeth but lightening results cannot be guaranteed.
  • The In-office Teeth Whitening procedure can sometime caused discomfort during the treatment. If you believe your tolerance or threshold level to pain is low, we strongly suggest taking Analgesics One hour before your Teeth Whitening appointment to our office
